Your primary care physician (PCP) has an entire team to help manage your health care needs. This team includes PCPs, physician assistants, nurse practitioners, registered nurses, medical assistants, complex care coordinators and licensed clinical social workers who will be able to partner with resources in the community to help you identify health goals that are right for you. This collaboration brings together the skills and expertise you need for personalized care from a team working towards one goal: a healthier you.
Your health and well-being are our priority. Our Primary Care Department is staffed with health care professionals who are here to take care for your health needs in the office, via video or phone visits, or electronically with an e-visit through MyChart™ by Hawaii Pacific Health.
