A. CareEverywhere allows your health information to be electronically shared between healthcare providers for treatment purposes. CareEverywhere provides continuity of care through the creation of a secure, virtual electronic medical record so you receive the best possible care no matter where you are treated.

A. The following participate locally:
- Hawaii Pacific Health, which includes:
- Straub Medical Center
- Pali Momi Medical Center
- Kapiolani Medical Center
- Wilcox Medical Center
- Kauai Medical Clinic
- Your provider or clinic if they use the Hawaii Pacific Health electronic health record in their practice
- The Queen’s Health Systems
- Kaiser Permanente Hawaii
Other health care provider organizations from across the country, including:
- Allina Health Systems
- Cedars-Sinai Health System
- Cleveland Clinic
- Johns Hopkins Medicine
- Kaiser, various regions
- Loma Linda University Health System
- Stanford Health
- Sutter Health Affiliates and Community Connect Practices
- And many more with the list growing regularly
In addition, the following Federal organizations participate on a limited basis:
- Social Security Administration
- Veterans Administration
Q. Why Does My Health Information Need to be Shared?
A. Health care providers can make better decisions about your care when they know about your medical history, lab test results, allergies, medicines, and other reports. Access to your health information saves you and your health care team both time and medical costs.

A. The information available depends on each participating organization’s electronic health record system.
There may be details of clinic and hospital visits including:
- Allergies
- Medical, Surgical, Family and Social History
- Diagnoses (including HIV)
- Test Results (including HIV)
- Medications
- Immunizations
- Procedures
Q. Is My Shared Health Information Kept Safe and Private?
A. Yes. CareEverywhere members obey all federal and state laws about medical information privacy. Organizations participating in the CareEverywhere system follow strict rules to protect the security and privacy of your health information.
Q. What Are My Rights? Can I Say No?
A. You have the right to decide whether your information is shared through the CareEverywhere system. You can choose to opt out of sharing information through CareEverywhere or to require that your consent be obtained for each release of information. You can change your mind at anytime.
