* Free helmets are for children while supplies last. Please have keiki first sign in at the registration table where they will be given a passport that will be stamped at various stations. Completed passports are eligible to enter a drawing to win a free bike.

Bike & Skateboard Safety Day Registration
To the Parent/Guardian:
Some Bike & Skateboard Safety Day activities will involve interactive participation and any and all risks associated with the activities are voluntarily assumed.
Parents/guardians are responsible to determine if their child is healthy and physically fit to participate in these activities. Parents/guardians must be present for children to participate.
If a child is injured during this event and their parent/guardian is not present, the child will be sent to the Emergency Department in an ambulance.
NOTE: By submitting this registration form, you are acknowledging that your child is healthy and physically fit to participate in these activities and that you understand and accept any associated risks, and are consenting to your child’s participation
