At Hawaiʻi Pacific Health, our dedicated team of patient navigators provides individualized care to help you through your cancer journey.
Help We Provide
Patient navigators are registered nurses, licensed social workers, and certified mammography technologists. These specialized professionals work with you and your loved ones to offer education, guidance, and resources so that you feel supported and empowered every step of the way.
Team members provide individualized education, guidance, and resources while coordinating care alongside your physicians and healthcare team to help ensure a seamless experience throughout treatment.
Depending on your individual needs, your navigators may offer the following:
Services
- Care coordination alongside your doctors and healthcare team.
- Educational materials and resources regarding your diagnosis and treatment.
- Referrals to support groups, homecare services, or other assistance services.
- Information about and access to clinical trials, genetic counseling, dietary counseling, and psychosocial support.
- Referrals to national support programs like the American Cancer Society's CARES program and Road to Recovery program, and the Look Good Feel Better program.
- Transition of care into our Survivorship Program.
