Biography
As a Board member of the Hawaii Academy of Family Physicians, former President and founding member of the state chapter of the American Medical Director's Association - Society for Post-Acute and Long-Term Care Medicine, Dr. Yazawa has a particular interest in improving transitions from hospital to skilled nursing home, rehab and community. He has been working in this field since 2001. He is Medical Director of several Skilled Nursing Facilities and Assisted Living Facilities. In addition, Dr. Yazawa is a West Point graduate, serves in the Army Reserves and has been deployed five times.
He enjoys Judo and Brazilian Jujitsu.

Personal Interests
Dr. Yazawa is a graduate of West Point and currently holds the rank of Colonel in the U.S. Army Reserves. He is also fluent in Japanese.
