CareEverywhere
Your health information can be shared between providers with CareEverywhere. It is a secure, virtual electronic medical record so you receive the best possible care no matter where you are treated.
Sharing health information helps health care providers make better decisions about your care. It helps them know more about your medical history, lab test results, allergies, medicines, and other reports. Access to your health information saves time and medical costs.
Organization Participation
The following participate locally:
- Hawaiʻi Pacific Health, which includes:
- Kapiʻolani Medical Center
- Pali Momi Medical Center
- Straub Benioff Medical Center
- Wilcox Medical Center
- Kauaʻi Medical Clinic
- Your provider or clinic if they use the Hawaiʻi Pacific Health electronic health record in their practice.
- The Queen’s Health Systems
- Kaiser Permanente Hawaiʻi
Other health care provider organizations from across the country participate, including:
- Allina Health Systems
- Cedars-Sinai Health System
- Cleveland Clinic
- Johns Hopkins Medicine
- Kaiser, various regions
- Loma Linda University Health System
- Stanford Health
- Sutter Health Affiliates and Community Connect Practices
- And many more with the list growing regularly
In addition, the following Federal organizations participate on a limited basis:
- Social Security Administration
- Veterans Administration
What is Shared
The information shared depends on each participating organization’s electronic health record system. There may be details of clinic and hospital visits including:
- Allergies
- Medical, Surgical, Family and Social History
- Diagnoses (including HIV)
- Test Results (including HIV)
- Medications
- Immunizations
- Procedures
CareEverywhere members obey all federal and state laws about medical information privacy. CareEverywhere organizations follow strict rules to protect the security and privacy of your health information.
Information Safety and Patient Rights
You have the right to decide whether your information is shared through the CareEverywhere system. You can opt out through CareEverywhere. You can also state that your consent be obtained for each release of information. You can change your mind at anytime.
