advance care planning.

You can complete an advance directive, learn about the important role of a health care power of attorney and know the difference between an advance directive and
Provider Orders for Life-Sustaining Treatment (POLST).

Facilitated by Advance Care Planning team.

  • February 28, 2019 (Thursday) from 4 - 6:30 p.m. at Pearlridge Conference Room, 98-151 Pali Momi St., Suite 142, Aiea, HI 96701 (Parking is available at covered parking structure behind clinic near Big City Diner).
  • March 23, 2019 (Saturday) from 9 - 11:30 a.m. at Internal Medicine Waiting Room, 46-056 Kamehameha Hwy, Kaneohe 96744 (validated parking at Windward Mall)
  • May 10, 2019 (Friday) from 8 - 10:30 a.m. at First Insurance Center, Hibiscus Conference Room 980, 862 South Beretania St., Honolulu, HI 96813 (validated parking at First Insurance Center Parking Structure)

Parking is validated.

A notary will be available for those who want to complete their advance directive with a valid picture ID. There is no cost for notary services.
