Advance Care Planning

SMC_ACP_Flier_092017_NC.JPG

Have you completed your advance directives?
Who is your health care power of attorney?
What is Provider Orders for Life-Sustaining Treatment (POLST)?

Advance Care Planning is the process of thinking about, talking about, writing down and sharing your own health care wishes. It is the greatest gift we can give to our loved ones.

Please bring your family and loved ones with you to discuss these important topics together.
