At present data shows low rates of advance care planning, clinical practice improvement, appropriate management of diagnostic imaging, proper complex care management, quality improvement, and primary care redesign, use of new innovations in prostate cancer screenings and care and chronic kidney disease. We also see high rates of physician burnout and frustration in our community and challenges with new updates. Various activities will address each of these gaps in knowledge and performance.



Objectives:
1. Discuss the data that identifies the gap.
2. List the evidence-based practices.
3. List the specific tools that are being made available to physicians and patients to address each gap.

Continuing Education:
In support of improving patient care, Hawai‘i Pacific Health is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

Hawai‘i Pacific Health designates this live activity for a maximum of 1.0 AMA PRA Category 1 Credit (s) ™, 1.0 contact hours for Nurses and 1.0 continuing education credits for Pharmacists.
