The purpose of this program is to improve the effectiveness of the POLST paradigm program through facilitation skills training and certification and improve the knowledge of RNs and Physicians. The skills include initiating POLST conversations, assisting in making informed end-of-life decisions, and learning techniques to create POLST plans.

Objectives:
1. Describe the key features of ACP program and define differences between AD and POLST.
2. Identify patients who would benefit from POLST discussions.
3. Demonstrate ACP and POLST interview skills thru group discussions.

Continuing Education:
In support of improving patient care, Hawai‘i Pacific Health is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

Hawai‘i Pacific Health designates this live activity for a maximum of 7.5 AMA PRA Category 1 Credit (s) ™ for physicians and 7.5 contact hours for nurses.
