Target Audience: Physicians/Nurses and other members of the patient care team. This offering is intended for HPH employees and credentialed HPH providers.

Learning Objectives: At the conclusion participants should be able to:
1. Identify performance improvement opportunities and best practices for optimal stroke care provision per the standards set forth by the AHA.
2. Explain the scope of resources available to participate in the acute care of the stroke patient at PMMC to achieve optimal patient outcomes.
3. Educate stroke providers as to changes in stroke care standards, expectations, goals and team roles.
4. Engage Inter-professional team members in a patient-centered discussion addressing needs which include palliative, psychosocial and rehabilitative care.
