Lymphedema Rehabilitation

Kapi'olani offers Hawaii's first comprehensive lymphedema rehabilitation program. Our certified physical therapists have been specially trained to perform Complex Lymphedema Therapy which entails special massage, bandaging, and education to reduce swelling. Kapi'olani will work with your doctor to design a treatment program to meet your needs.

Our goal is to provide you with the knowledge and skills to manage lymphedema on your own. In addition to Complex Lymphedema Therapy, we offer free monthly lymphedema information sessions.

If you notice signs of lymphedema, see your doctor first. Your physician can refer you to Kapi'olani as appropriate. We will work closely with your doctor through follow-up phone calls and written reports. You can reach us at (808) 485-4415.

What is Lymphedema?

Lymphedema is the chronic swelling of some part of the body due to malformation, damage or removal of part of the lymphatic system.

Lymphedema is fairly common in the U.S., affecting more than one percent of the population. It usually affects an arm or leg, but can also occur in the face, neck, abdomen or other areas.

What is the Lymphatic System?

The lymphatic system consists of lymph vessels and nodes, and the tissues they serve.

Lymph is the fluid carried through the lymph vessels. The fluid drains waste products from the body tissues.

This fluid is filtered through lymph nodes, which purify the lymph of waste matter and bacteria before it returns to the bloodstream.

In a normal lymphatic system, swelling from minor trauma is temporary and clears in a few days.

However, if the lymphatic system has been removed, blocked or damaged (as with cancer treatment), the vessels are unable to drain the fluid, and lymphedema occurs.

Primary lymphedema is caused by malformation of the lymphatic system. The abnormality may be present at birth or can occur during adolescence or later in life.

Secondary lymphedema occurs when the lymphatic system is damaged or a part of it is removed, as with radiation or cancer surgery. This type of lymphedema can arise any time following the treatment.

What are the Signs?

Initially, you may notice swelling. A watch or ring may feel tight. Sometimes the limb has a sensation of "pins and needles" or heaviness.

Unusual or persistent swelling should always be evaluated by your physician.

Is There Anything I can do to Prevent Lymphedema?

Anyone who has undergone cancer treatment is susceptible to lymphedema. However, there are precautions you can take to minimize your risk.

Avoid infections:

  • Avoid cuts or scratches (use an electric razor)
  • Use protective gear, such as gloves when gardening or washing dishes, etc.
  • Avoid insect bites or pet scratches
  • Use care with manicures and pedicures
  • Take injections or blood tests on your healthy arm

Avoid trauma or injury:

  • Have blood pressure taken on your healthy arm
  • Don't wear tight jewelry or clothing
  • Don't overexert the arm with excessive exercise or work

Avoid temperature extremes:

  • Use caution with hot tubs, saunas and steam baths
  • Use sunscreen with SPF of 15 or higher
  • Use care when traveling to excessively hot climates

General precautions:

  • Maintain meticulously clean skin to avoid infection
  • Use lotion or moisturizers
  • Maintain a healthy diet
  • Watch your weight
  • See your doctor at the first sign of infection
  • Use caution when flying

Can Lymphedema be Cured?

Damage to the lymphatic system generally cannot be repaired. However, lymphedema can be controlled. Fluid can be moved from the swollen area to a part of the body where the lymphatic system is working. The fluid can then drain back to the bloodstream.

If you think your arm or leg is swelling, use the chart below to track it. Measurements should be taken on three consecutive days at the same time each day. Have the same person help you. The picture to the right of the chart below indicates the areas to measure. Measure both the affected and unaffected arm or leg.

If there's a difference from one day to the next, call your doctor, who can advise if the change is significant and if further examination is necessary.
