HealthAdvantage: A New Approach to Care
At Hawai‘i Pacific Health, keeping you healthy is our goal. That’s why we’ve established HealthAdvantage, a new way of delivering health care that is focused on wellness and preventive care. You may have heard the term “Patient Centered Medical Home” and that’s what HealthAdvantage does – it partners patients with their primary care physician (PCP) and an extended team of nurses and health educators who are all focused on good health. Along with your insurance health plan, we are working together to make sure you get the right care, when you need it.
Diabetes costs the United States nearly $174 billion per year and is expected to triple by 2035. In Hawai‘i, caring for people with diabetes now costs $1 billion. Native Hawaiians, Japanese and Filipino residents 20 years or older are 200% more likely to be diagnosed with diabetes. Individuals with diabetes are twice as likely to experience disability due to amputation, vision loss and other seriously limiting medical conditions. Diabetics are also at greater risk for stroke, heart attack, kidney failure, nerve damage, severe dental disease and pregnancy complications.
At Hawai‘i Pacific Health, we are seeing remarkable results with diabetic patients who are a part of HealthAdvantage. In just two years, using a pilot population of 50,000 people across the state, our patients are showing significant health improvements in key diabetes indicators such as A1C (blood glucose level), cholesterol and blood pressure. Through the use of our electronic medical record system (EMR) that seamlessly connects all of our physicians and hospitals, our care teams can track the health of a large patient population and quickly identify at-risk patients and get them into the office for immediate treatment. In fact, Kaua‘i Medical Clinic and Straub Clinic & Hospital have done such a tremendous job of caring for their diabetic patients that their results put them in the top 10% percentile nationally in nearly all of the quality measures.

MyHealthAdvantage is an online tool that patients can access 24 hours a day, seven days a week from any computer, iPad, iPhone and Android device. MyHealthAdvantage helps patients manage their health by giving them access to portions of their EMR anytime, anywhere. Patients can conveniently:
- Schedule medical appointments
- View test results
- Request prescription refills
- View portions of their health record
- Receive reminders of upcoming tests
- Send secure messages to their doctor
- Track their blood pressure and other health measures
